For some people, it's the exact opposite, as I said. They like the quests in the island, then they reach the mainland and are lost and confused about what they are supposed to be doing. Most often, the "whatever you want" answer doesn't satisfy them entirely (and we have learned recently that "whatever you want" can be potentially harmful to the community as well).
In both cases, the island doesn't give an exact representation of what gaming on the mainland will be : no quests on the mainland will give you XP like on Silan. Most of them will only reward you in fame and/or dappers or civilisation points tradeable for items. Some of them will unlock rites that reward with special abilities, such as more focus, more durability on crafts, special auras, more flags, special generic craft plans... no one has to do them, they are just a possibility among many.
For the rest, it's grind as usual, but most games are just that. Repetitive ? Well, as if raiding an instance for the 25th time in the hope that MAYBE that rare piece of loot will drop wasn't ... well we don't have those instances and ugly drop rates, but it just as repetitive as you make it to be.
Many reasons stated in the past, for why people grew bored and left. The only reason I keep returning, is that when you've made your place in people's memories, here, they welcome you back with a hug and a smile when you return... well... most of them.
