4x250 meant a character that hit 250 in each tree, regardless of branch. However, now I am a little confused. Once you pick a branch, can you go back and change it, or improve in a different branch simultaneously?

You start off with 4 general skills in the tree..then they split up and let you specialise..

every action you do gives you xp on a single branch, but branches share a commoon trunk. According to your level, the xp can be given to "melee fight" or "master 2h axe". When you mean simultaneously, yes, you can for example, use a sword to kill a few (thus getting xp in sword) and then a pistol, or even some acctions will give you xp in 2 like "afflicting missile"
