More missions from different NPCs, I think. Higher fame, more NPCs care to give you missions. I've roamed Pyr quite a bit, and I can see NPCs who have a mission box, but nothing to give out.

Fame is key to getting guild missions.

Fame also effects interactions with NPC merchants and guards.
